Organ Donor Program

The organ donor program offers life to thousands of people.

If you would like an organ and tissue donor designation—DONOR♥—on your driver license or identification card, please check the appropriate box on your Driver License/Identification Card Application or when ordering a duplicate driver license / identification card on ServiceArizona.com. Standard transaction fees apply.

If you already have the DONOR♥ designation on your driver license or ID card and would like it removed, you must go to any MVD office and make the request. A new license or ID will be mailed to you without the designation. Standard transaction fees apply. To remove your name from the DonateLife AZ Registry, contact Donor Network of Arizona.

Pursuant to Title VI of the Civil Rights Act of 1964, and the Americans with Disabilities Act (ADA), ADOT does not discriminate on the basis of race, color, national origin, age, sex or disability.
