1. Print the form and then write in your information, or
2. Type your information into the fields provided on the form. After selecting the form, click in an appropriate field to make an entry. Use your tab key to advance to the next available field. When your are finished, you can print the form.

Please Note: Permits can only be purchased as follows:

1. EZ Pay accounts
2. In person at Arizona Commercial Permits or Ports of Entry
3. Using a National Permit Service
