Before a Class C Permit can be purchased, you must first request a Class C Approval. To request a Class C Approval, please follow the procedures listed below.
Please read carefully prior to submitting request form.
- Complete the Class C request form (please type or print). Ensure request form is complete and accurate. Incomplete or inaccurate information on request forms causes delays in processing approvals and issuance of a permit
- Ensure that the company name is the same each time a request form is submitted.
- When obtaining permits, ensure that the dimensions, weights, truck unit numbers, etc. are the same as what was submitted for a Class C approval.
- Fax the Class C request form to 602-712-3380.
- Loads over 250,000 lbs. or overaxle weight are reviewed by the ADOT Bridge Operations section. Analysis could take up to two weeks.
- We can only forward Class C approvals to a port of entry or Goodyear/Central, Tucson, Safford, Sierra Vista, Willcox or Benson office. Note on request form where approval is to be sent.
- The Class C approval is not a permit. Permits must be obtained through a port of entry or the Goodyear/Central, Tucson, Safford, Sierra Vista, Willcox or Benson office.
- Administrative fee for Class C approval is in addition to the permit fee. ($15.00 for loads up to 18 feet in width or height, $25.00 over 18 feet). Additional fees may be required for certain bridges.
- Class C approval is valid for five working days. An extension of time on an approval must be requested prior to the expiration date of an approval and before purchasing a permit.
- Class C approvals/permits are valid on state and interstate highways only.
- Normal time frame for the completion of a Class C approval is three days (72 hours, excluding weekends and holidays from the date and time the request is received). However, most approvals are completed within one to two days. To check if approval is completed earlier than the three days, please contact the port of entry or office where you indicated approval was to be sent.
NOTE TO ALL PERMIT COMPANIES: When submitting a Class C request form, it is necessary to provide the phone number of the transporter.
