Organ Donation -
There is a new way to indicate your decision to be a donor.
Beginning December 31, 2005, the application for a driver license or identification card will contain a check-off box for you to request the release of your name and address to the Arizona Donor Registry. When you check this box, registration information will be mailed to you. You may also register to be an organ donor at anytime by visiting www.servicearizona.com and clicking on the Donate Life logo, or by calling 800-94-DONOR for a registry form.
Legislation to create an official donor registry was passed in 2002. Prior to this, there was never an official way to register your wishes to be an organ and tissue donor in Arizona. The Arizona Donor Registry provides a centralized place to document your wishes to save and enhance lives through organ and tissue donation.
Registration information will be mailed to everyone who checked “Yes” on their license prior to the implementation of the Arizona Donor Registry. If you have any questions about the Arizona Donor Registry or organ donation, please contact the Donor Network of Arizona at 800-94-DONOR.
