General Information

The Medical Review Program helps ensure that CDL drivers meet Federal Regulation 49 CFR 391.41 Physical Qualifications for Drivers and CFR 391.43.
We review the DOT Medical Examination Report (long form) and Medical Examiner Certificate (cert card or short form) for completeness and ensure the medical examiner has qualified the driver according to Federal Motor Carrier Safety Administration (FMCSA) Regulations.
CDL drivers are required by law to keep a current DOT Medical Examination Report and Medical Examiner Certificate on file with MVD. The DOT Medical Examination Reports are to be renewed every 24 months or less, as determined by the medical examiner. A courtesy expiration notice is sent to the driver 45 days prior to the expiration date.
It is the responsibility of the driver to provide us with a new DOT Medical Examination Report and Medical Examiner Certificate on or before the expiration date of the document currently on file.
If you no longer wish to keep your CDL, you must go to an MVD office and downgrade to a regular Class D operator license or Class M motorcycle license prior to the expiration of the current Examination Report, to avoid a suspension and/or revocation. Application fees will apply.

Additional Requirements

If you receive a request for additional information this means your Examination Report was incomplete and/or you require restrictions to be added to your license. Please note only the boxes marked apply to you. Corrections to your Examination Report must be initialed and dated by your physician.

Any request for restrictions to be added to your license must be done at a CDL office A duplicate license fee will be charged.
|
|
|
If you do not submit the requested information in a timely manner, your CDL privilege will be suspended and/or revoked. If you become suspended and/or revoked, the requested information may be faxed to 623-925-9323 or e-mailed (see Contact Medical Review)
You must visit a CDL office to reinstate and pay any applicable fees.

Suspension and Revocation
|
|
Reasons for receiving suspension and revocation notices:
- Failed to submit a current DOT Medical Examination Report and Medical Examiner Certificate.
- Failed to submit requested additional information and/or add restrictions.
- Failed meet medical and/or vision standards to operate a commercial motor vehicle. You may call 602-771-2460 for further information.
If your CDL privilege is suspended and/or revoked for failure to submit your DOT Medical Examination Report, Medical Examiner Certificate or additional information, you may submit your information by fax to 623-925-9323 or by e-mail (see Contact Medical Review).
You have the right to request a hearing on these actions, however, such request will not stop the immediate suspension. Your request must be received within 15 days from the date of the notice. The request must be made in writing and may be made by any one of these methods:

The request must include your full name, date of birth, license number, mailing address, home and business phones, and Case Number. Upon receipt of your written request, a hearing may be scheduled and you will be notified as to the date, time and location.
If your CDL is disqualified, suspended, or revoked for one year or more, the CDL is considered to be expired (invalid). You will be required to take all written and skills tests for CDL, and pay all applicable licensing and reinstatement fees.

Intrastate Vision Waiver (to drive in Arizona only)
|
|
Requirements
You must have driven the type of vehicle to be operated as stated in the waiver, for at least two of the previous five years. You are prohibited from transporting passengers or hazardous material.
To Apply
You must submit a copy of a current DOT Medical Examination Report, Medical Examiner Certificate, a current Vision Examination Report completed by an ophthalmologist or optometrist and a completed Intrastate Waiver Application.
If you are found to be eligible for the waiver, you will be required to take the Special Performance Evaluation (SPE) road/skills test at an MVD CDL office with a certified examiner prior to issuance of the waiver.

Intrastate Limb Impairment/Amputation Waiver (to drive in Arizona only)

To Apply
