All applicants must use the new Material Source Environmental Analysis Application form to complete the ADOT Environmental Analysis process in accordance with the 2000 edition of Standard Specifications for Road and Bridge Construction Section 104.12. As a reminder, state, vicinity, and site maps are required with each application. Be sure to indicate the haul road on the vicinity and site maps. Please note the cultural survey and other questions, such as the one pertaining to Threatened & Endangered Species, must also cover the haul road, in addition to proposed material source area.
